A Kurri Kurri man, 74, died in John Hunter Hospital a short time after his female passenger, 40, died at the crash scene.
The accident occurred when the white Hyundai Excel, being driven in an easterly direction by the elderly man, collided with a troop carrier before spinning into the path of a Mitsubishi Triton 4WD.
The crash scene was about 200 metres west of the George Booth Drive roundabout and in a restricted 40kmh zone.
The road was closed to all traffic for about 7 hours, reopening at 4pm.
The Hunter Westpac rescue helicopter had earlier flown the man to John Hunter Hospital with critical injuries but he later died.
The drivers of the four-wheel drive vehicles were not seriously injured and are assisting police with their inquiries.
